Care in the home

The Natick VNA provides acute care to patients recovering from hospitalization in their home through skilled nursing, therapy and rehabilitation, medical social work, telehealth, palliative care and prescription drug assistance. For individuals who need this type of clinical care, we are able maintain already existing relationships between medical practitioners and their patients throughout the course of that care. In fact, a recent study found that 91% of Natick VNA patients would recommend the agency to others.

Fostering innovation

The Natick VNA has a distinguished track record of innovation. We were the first home health agency in Massachusetts to use laptop computers to record patient clinical information. As a result, we have saved time, paper, and storage space, and we continue to improve our ability to communicate with doctors, labs, and other health team members. We are proud to have adopted the first totally web-based integrated clinical-financial documentation, billing, and reporting system in the country.

We were among the first to begin using a home telemonitoring system, which allows us to monitor patients’ weight, vital signs, and general health status daily. The system helps us to decrease emergent care and hospitalization for our patients and improves their home-based recovery and health maintenance.

Home care services are available seven (7) days a week, 365 days a year.

Office Hours:

Monday – Friday: 8am – 5pm

Weekends & holidays: 8am – 3pm

Patients experiencing medical issues may contact us twenty-four (24) hours a day.
